Conduct comprehensive assessments of clients; strengths, challenges, and goals related
to their mental health recovery.

Develop individualized psychiatric rehabilitation plans that outline clients; goals, objectives, and strategies for skill development.

 Assist clients in building and enhancing life skills, including communication, problem-
solving, coping, time management, and socialization.

Facilitate group sessions to promote skill-building, mutual support, and community
integration among clients.

Collaborate with case managers, therapists, medical professionals, and other team
members to ensure coordinated and holistic care.

Provide psychoeducation to clients and their families about mental health conditions,
coping strategies, and wellness practices.

 Offer crisis intervention and immediate support to clients in emotional distress or crisis
situations.

Monitor clients; progress and adjust PRS as needed based on their evolving needs and
achievements.

Empower clients to set and achieve their recovery goals, promoting a sense of
autonomy and self-advocacy.

Maintain accurate and confidential client records, documenting progress, interventions,
and outcomes.

Participate in interdisciplinary team meetings and case conferences to share insights
and contribute to clients; care plans.
