MANAGER
SUMMARY
The targeted case manager at our community behavioral health services agency is essential in supporting individuals facing mental health challenges. They coordinate care, advocate for client needs, and ensure access to necessary resources. This role emphasizes building strong relationships, developing tailored care plans, and collaborating with a diverse team to enhance clients' overall well-being. RESPONSIBILITIES

Conduct comprehensive assessments to understand clients, mental health needs, strengths, challenges, and goals.

Collaborate with clients to develop personalized care plans that address their individual needs and promote their recovery.

Coordinate and monitor services across multiple providers, including psychiatrists, therapists, medical doctors, and community resources.

Serve as a central point of contact for clients and their families, providing guidance and support in navigating the mental health system they receive timely and appropriate care.

Advocate for clients; needs within the healthcare system and the community, ensuring they receive timely and appropriate care.

Assist clients in accessing community resources such as housing, employment support, financial assistance, and social services.

Provide crisis intervention and support to clients during emotional distress or crisis situations referrals.

Regularly assess clients; progress toward their goals and adjust care plans as needed.

QUALIFICATIONS

Bachelors or master´s degree in social work, psychology, counseling, or related field.

Relevant licensure or certification, if required by state regulations.

Experience in case management or care coordination, preferably in a mental or behavioral health setting.

Strong interpersonal skills to establish client rapport and effectively communicate withdiverse individuals and teams.

Knowledge of community resources and services to connect clients with appropriate support systems.

Ability to work collaboratively within a multidisciplinary team and advocate for clients;needs.

Excellent organizational skills to manage multiple client cases and maintain accurate documentation.

WORK CONDITIONS
The targeted case manager will work within a community behavioral health services facility,
often engaging in person and remote client interactions. The role may require some travel to
meet clients or attend meetings, and flexible hours may be necessary to accommodate clients schedules.
